Primary, Third & Booster Vaccination
Primary Dose
Primary vaccination consists of:
- two doses of Comirnaty (Pfizer) vaccine three weeks apart
- two doses of Spikevax (Moderna) four weeks apart
- two doses of Vaxzevria (AstraZeneca) vaccine 4-12 weeks apart
- one dose of Johnson & Johnson (Janssen) vaccine
Comirnaty (Pfizer) is approved for use in children 5 to 11 years of age.
Comirnaty (Pfizer) and Spikevax (Moderna) are approved for children 12 years of age and older and adults.
Vaxzevria (AstraZeneca) and Johnson & Johnson (Janssen) are approved for people 18 years of age and older.
Third Dose
In immunocompromised individuals, the primary vaccination consists of three doses while two doses are recommended for healthy individuals. This means that an immunocompromised person should, after the second dose of Comirnaty (Pfizer) or Spikevax (Moderna), receive a third dose of the same vaccine.
An immunocompromised person who has received two doses of Vaxzevria (AstraZeneca) should receive a third dose of an mRNA vaccine (Comirnaty/Pfizer or Spikevax/Moderna). No additional doses are recommended at this time.
For immunocompromised persons who have received the Johnson & Johnson (Janssen) vaccine, revaccination is mandatory.
The recommended interval between the second and third dose of primary vaccination in immunocompromised individuals is eight weeks, but can be a minimum of four weeks.
Booster Dose
There is growing evidence of weakening protection against infection after receiving the primary vaccinations, regardless of the type of vaccine used. Furthermore, the weakening of protection from infections has been observed in all age groups. In addition, there is growing evidence that an additional dose improves the level of protection. Therefore, revaccination (or booster) against COVID-19 is recommended.
For all those who have received the Johnson & Johnson (Janssen) vaccine as a primary vaccination, revaccination/booster is recommended at least two months after the single dose. For this purpose it may be Johnson & Johnson (Janssen) or an mRNA vaccine (Comirnaty/Pfizer or Spikevax/Moderna).
It is recommended that people over 18 years of age, who have completed their primary vaccination with Spikevax/Moderna,
Comirnaty/Pfizer or Vaxzevria/AstraZeneca, be revaccinated/get a booster with a single dose after six months from the primary vaccination.
However, it is not incorrect to get vaccinated before six months has passed. A third dose which is administered at least three months after the second dose is recognized as a booster dose.
People who have been primarily vaccinated with Comirnaty/Pfizer or Spikevax/Moderna should be vaccinated with the same vaccine. A person who has been primarily vaccinated with Vaxzevria/AstraZeneca should be vaccinated with one of the approved mRNA vaccines (Comirnaty/Pfizer or Spikevax/Moderna).
IMPORTANT NOTE: For revaccination Spikevax/Moderna uses half the dose used for primary vaccination (0.25 ml).
Booster doses are especially recommended for:
– residents of homes for the elderly and infirm and for adults with disabilities.
– workers employed by accommodation service providers for the elderly and infirm, adults with disabilities and by other accommodation service providers.
– healthcare professionals who directly care for patients.
– people aged 50 and over, regardless of comorbidities.
– people aged 18 and over who share a household with immunocompromised and highly vulnerable people.
– everyone aged 18 and over suffering from chronic diseases which increase the risk of more severe forms of COVID-19.
Second revaccination (so-called fourth dose) for people over 80 and people over 65 who are residents of collective accommodation (homes for the elderly and infirm).
So called. the fourth dose should be given at least four months after the first dose. The mRNA vaccine (Pfizer or Moderna) is used for this purpose. Also, a second vaccination is recommended for people who are at increased risk of developing severe COVID-19, who have been primarily vaccinated with the Janssen vaccine and have been vaccinated with the Janssen vaccine.
Vaccination for People Who Have Recovered from COVID-19
It is strongly recommend that you receive at least one dose of the vaccine least six months after recovery, and it is acceptable to get the entire primary vaccination, in accordance with the recommendations under point 1 (primary dose), with possible larger intervals between doses.
It is not incorrect to receive the vaccine even with an interval of less than six months. Unfortunately, the rules for issuing digital COVID certificates are not in line with expert recommendations. Therefore, if a person wants to get a digital COVID certificate based on a single dose after recovery, this single dose should be received within 180 days of a positive PCR test.
People aged 65 and older and people with moderate and severe immunosuppression who had COVID-19 should receive the vaccine at least three months after recovery in accordance with the recommendations under point 1 (primary dose).
Revaccination is not recommended for people who have been fully vaccinated (received the primary doses of the vaccine) after recovery. A second dose of vaccine is recommended for people who recovered from COVID -19, and received one dose of vaccine after recovery, provided that six months have passed since receiving the vaccine.
Note: Recommendations are subject to revision. Original links here and here.
